Methodology

We retrieve the published datasets from Centers for Medicare & Medicaid Services (CMS) directly, keep an immutable dated copy of every file we retrieve, and build these pages from that copy.

• Every page shows the as-of date of each dataset behind it.
• Findings are shown as the authority wrote them. Any plain-English explanation appears beside the official text, never in place of it.
• Where a facility has formally disputed a citation, we say so on the page.
• A finding with no source text is not published at all.

A record here reflects what the authority published on the as-of date. Facilities change, and records are corrected. Always check the authority for the current position before acting.
